If you would like a copy of your medical record print the Release of Medical Records from the link below, complete your details and fax your submission to us with a copy of your photo ID. Turnaround time on these requests is two weeks. The fee for patients is $45.00, and can be paid at the time you pick up your medical records. Please make sure to bring or fax your picture ID, we are unable to release you records without it.
